The Youth Participation and Access program funding agreement will detail the program provider’s role in the delivery of programs. This includes details on reporting requirements and key milestones (Word 71kb) as well as acknowledging the YPA program.

YPA Performance Report - 1 January 2008 to 30 June 2008

YPA program providers are required to submit a six-month YPA program Performance Report to the Office for Youth in January and July of each year of funding. Providers are required to report against the information provided in the YPA Annual Program Plan and Activity Summaries that were submitted for 2006/2007.

To align with the Annual Program Plan and Activity Summaries, Performance Reports have been separated into 2 parts:

Part 1 General Report (Word 144 kb) - This report collects information on the whole of your YPA program.
Part 2 Activity Report (Word 134 kb) - This report is completed for each YPA Program Activity Summary.

Completing and submitting your reports

• Download reports from above.
• Complete and submit electronically according to the Instructions for completing YPA Performance Report (Word 142 kb).
• Please email your Performance Reports by 31 July 2008 to ypa@dpcd.vic.gov.au.
• The Office for Youth will provide confirmation by email upon successfully receiving your Performance Reports. Please contact the YPA Project Officer if no contact has been made within 2 weeks of submitting your reports.

Annual Acquittal 2007-08

YPA Program Providers are required to complete an Annual Financial Acquittal. The next acquittal is due on 31 July 2008.

YPA Promotion

The Victorian Government and the Youth Participation and Access program must be acknowledged in any publication, promotional material or news releases. You should use the State Government logo and acknowledgement when you are producing materials such as presentations, flyers, promotional material, information sheets, brochures, information to launches etc.
